Fred died in AtS episode 5.15, "AHoleInTheWorld A Hole in the World?". After breathing dust from a mysterious sarcophagus that was delivered to her lab at WolframAndHart, she was taken ill. The MoG discovered that Fred's internal organs were being liquified in order for her body to be taken over by the ancient being {{Illyria}}.
The sarcophagus was originally kept in the Deeper Well, located at the Cotswolds in England, but was pre-ordained to reappear in LosAngeles. Before arriving at W&H, it had been held up in customs, but ironically, it was freed by CharlesGunn, who was unaware that actions would help lead to the death of his former girlfriend.
Fred's final words were, "Wesley, why can't I stay?". Moments later, {{Illyria}} stood up wearing Fred's body.
It was revealed in AtS episode 5.16 "{{Shells}}" that Fred's soul had been destroyed resurrecting {{Illyria}}.
